Adult Mental Health Care

Our goal is to take care of the “whole person” via a person centric care plan approach. Our therapeutic rehabilitation offers service users the ability to maintain and gain daily living skills. Most of our Service Users have been long standing occupiers of continuum beds in hospitals who may be/have experienced psychiatric, psychological and emotional problems, including the severely ill who require high dependency or intensive care and may be at risk of suicidal behaviour, serious self-harm, violence or absconding.

We offer a move-on facility from Acute Wards (Public & Private Hospitals) to community based living via three stages; Intensive Recovery, Supported Recovery and Supported Living to Community Living.
Via a highly tailored and objective care plan we reduce relapses and future hospital admissions, thus reducing the reliance on state and independent psychiatric hospitals for acute care.

Priory Manor offers a long term rehabilitation and continuum care package that is highly personalized for Service Users with enduring mental illness and behavioral difficulties and challenging behaviors who may be occupying continuum care beds in the NHS or require move-on from Independent Hospitals
The therapeutic rehabilitation program is founded in a broad conceptual approach which includes;

Restoration and maintaining current skills, self care with an aim for independent living.
Socialization and community integration (if applicable)
Development of new age appropriate skills
Prevention of relapses
Recreation & Access to Educational Courses

During the first 12 weeks our multi-disciplinary team conducts and in-depth assessment of the Service User. This document clearly sets out the aims and objectives for slow stream rehabilitation. Via regular reviews we will be encouraging development of the service user for re-settlement into the community Supported Living program that is on offer in Plymouth and Launceston.
