empowers and enables a climate of informed choices, enhancing the capacity of clients to function independently within the community. To fully commit to the building of a community that is characterized by understanding and acceptance of those with Mental Health and dual diagnosis issues.

Our goal is to improve mental, physical, social and

cognitive function through rehabilitation and therapy, enabling clients to acquire, maintain and practice the necessary skills to manage and maintain independent living and to ultimately achieve a greater independence by living on their own within the community.

Our expertise for clients receive care from

interdisciplinary teams of experts both in-house and through our network of Specialist Support Services.

AMHC Identify and facilitate appropriate continuation of rehabilitation programmes moving from Intensive Recovery, Supported Recovery, Supported Living eventually helping the Service User into their own independent homes.

practices, model programmes, innovation and research and continual quality improvements.

 

Continuum of Care:

  • Continuously promote the quality of care, through comprehensive array of flexible living support including rehabilitation, therapy, early intervention, intensive case management, residential treatment, crisis intervention and self help services and also to provide links to other health and social services within the community.

We deliver innovative, seamless, client centred, comprehensive and rehabilitative dual diagnosis and mental health support services. We recognize the need for all stakeholders to work cohesively to create accessible and appropriate quality solutions for every client in our homes. The provision of these services through our participative models of care which integrate the results of continuous research and input from partnerships throughout the community.
